New decisions need to be made about work, housing, and social life, often all at once. We help young people and their families plan for this transition early, building the capacity and confidence needed to step into adult life on solid ground.
Into Independent or Supported Living
Whether you are leaving the family home for the first time or moving between living arrangements, we provide the practical planning and capacity-building support to make the transition smooth, prepared, and genuinely your choice.
Changes in Health or Functional Needs
When disability changes or a health event alters what support looks like, the existing plan needs to change with it. We help you and your support coordinator navigate that adjustment, building your capacity to manage the new situation and maintain your independence through the shift.
Moving to Support 24x7 From Another Provider
Changing providers is a transition too. We coordinate carefully with your previous provider, ensure continuity for any critical supports, and communicate with you and your support coordinator at every step. You should never experience a gap in support because of an administrative handover.
